Understanding why some patients recover better than others after ACL surgery: the ACHILLES Study

Anterior cruciate ligament (ACL) injuries are common and often require reconstructive surgery. While many patients recover well, growing evidence suggests that a subgroup experience an abnormal or dysregulated immune response to injury and surgery. These patients may have poorer recovery outcomes and could be at greater risk of developing osteoarthritis in the future. However, the biological mechanisms driving this response are not yet fully understood.

Project aims

In our Anterior Cruciate Healing Inflammatype(s): a Longitudinal Exploratory Synovial (ACHILLES) Study, involving experts from the University of Birmingham and Keele University, we aim to investigate whether tissues within the lining of the joint contribute to this altered immune response. Building on observations from both ACL surgery and other orthopaedic procedures, the project is exploring why some patients experience a prolonged inflammatory response while others recover more successfully.

Over the course of the study, researchers will collect joint fluid, synovial tissue biopsies and blood samples from up to 20 patients undergoing ACL reconstruction surgery. Samples will be collected at the time of surgery and again six weeks later to provide insights into how the immune response changes during recovery.

Using advanced protein analysis techniques, the team will examine a wide range of molecules involved in inflammation and immune regulation. Detailed analysis of synovial tissue will help identify the specific cell types responsible for these changes and determine how they contribute to recovery outcomes.

By improving understanding of the biological processes that influence healing after ACL injury, the ACHILLES Study aims to identify new targets for treatments that could help reduce harmful inflammation and improve surgical outcomes.
