New study uses The Simpsons universe to explore link between gum disease and all-cause mortality

A new, tongue-in-cheek study, led by researchers at the University of Birmingham, has found that recurring characters in The Simpsons who experience gum disease or tooth loss are significantly more likely to die than those with healthy teeth.

Published in the Journal of Periodontal Research, the researchers’ findings add another unlikely prediction to the long-running show’s remarkable track record of ‘foreshadowing’ real world events.

The researchers examined 150 recurring human characters from Springfield across more than three decades of episodes – investigating whether characters with signs of periodontitis or tooth loss were more likely to experience a canonical on-screen death.

After adjusting for factors including smoking, obesity, income, ethnicity and sex, the researchers found that characters with periodontitis or tooth loss had a 23-fold higher risk of mortality than those without these conditions. Smoking and abdominal obesity were also associated with increased mortality risk within the animated universe, mirroring real-world risk factors.

The study was inspired by one of Springfield’s most memorable characters, jazz musician Bleeding Gums Murphy, whose death aired in 1995 – making history as the first recurring Simpsons character to die on screen. The researchers noted that his ‘death’ predates one of the first real-world studies linking periodontal disease and mortality, which was published in 1998.

“When we realised that Bleeding Gums Murphy’s death appeared three years before one of the first published studies linking periodontal disease and mortality, we couldn’t resist asking whether The Simpsons had ‘done it again’ by forecasting another accurate, real-world prediction. The findings are, of course, entirely fictional and should not be used to guide clinical practice in either our universe or the Simpsons’. However, our study uses humour to shine a light on a very real and important area of research: the relationship between oral health, inflammation and wider systemic health.”

However, our study uses humour to shine a light on a very real and important area of research: the relationship between oral health, inflammation and wider systemic health.”

The researchers screened 200 characters from the series before applying a set of exclusion criteria that removed non-recurring, non-human, fictional-within-fiction and historical characters, whittling this list down to 150 recurring characters that were examined. Data on oral health status, mortality and potential confounding factors were gathered from publicly available sources, including fan-maintained databases and episode archives.

While the analysis produced striking results, the authors are clear that the study is solely intended as a humorous exploration of the real-world challenges involved in researching the links between oral and systemic disease in humans and potential mortality rates.

Professor Thomas Dietrich, Professor of Oral Surgery at the University of Birmingham and co-author of the study, said: “Behind the humour is a serious message: there is substantial evidence that oral health is closely connected to our general health, but studying those relationships is complex. By taking the question into an animated universe, we were able to illustrate some of the methodological challenges researchers face in a way that is hopefully accessible and engaging.

Dr Praveen Sharma added: “In our universe, the next step is better, more careful research into periodontitis and its implications for our wider health – using robust definitions, appropriate outcomes and sound statistical methods. If this paper encourages people to take a greater interest in oral health or helps researchers understand and explain why these studies are so difficult to conduct, then it has achieved its purpose.”

The authors conclude that while the periodontal-systemic health links observed in the Springfield universe are entirely coincidental, the study reinforces public awareness of the importance of oral health and demonstrates that inspiration for scientific enquiry can sometimes come from unexpected places.
